Public vs private insurance: key differences

Germany offers two main healthcare coverage options. These are the most important differences.

🏛️ Public insurance (GKV)

  • Cost: Percentage of gross salary (~17.5% total in 2026, split equally between employee and employer)
  • Family coverage: Non-earning spouse and children included at no extra cost
  • Access to specialists: Standard waiting times apply
  • Flexibility: Limited, standard coverage regulated by the state
  • Hospitalisation: Shared room by default
  • Who can take it out: Compulsory for employees earning below the annual threshold

🏆 Private insurance (PKV)

  • Cost: Fixed premium based on age, health status and coverage — does not increase with salary
  • Family coverage: Individual payment per member, but personalised coverage options
  • Access to specialists: Priority access with much shorter waiting times
  • Flexibility: High — you choose exactly what coverage to include
  • Hospitalisation: Single or double room, head of department doctor
  • Who can take it out: Employees earning over €6,450/month or self-employed workers
